The limitations of stress testing are well documented. A standard stress test is designed to detect blockages that narrow an artery by 70 percent or more, according to Harvard Health Publishing, which notes heart attacks frequently result from smaller blockages that rupture rather than the severe narrowing the test is built to catch. A normal result cannot rule out that kind of rupture occurring later, explains the center, which is why the U.S. Preventive Services Task Force and the American College of Cardiology do not recommend routine stress testing for asymptomatic people. Furthermore, a landmark study in The Lancet found nine modifiable risk factors, including cholesterol, blood pressure and smoking, account for roughly 90 percent of the population-level risk of a first heart attack, meaning that stress testing should only be considered as one part of a broader diagnostic process.

The NY Center for the Prevention of Heart Disease's approach combines Coronary CT Angiography with Cleerly and HeartFlow artificial intelligence analysis, along with advanced cholesterol testing through Boston Heart Diagnostic Labs, to look for plaque and risk markers before symptoms develop. According to the center, standard cholesterol testing misses over half of those at significant risk for a heart attack or stroke, and over half of heart attacks occur as the first sign of disease, without warning. This, again, stresses the need for broad-spectrum testing to provide accurate risk levels.

The center is led by Dr. Bradley Radwaner, a board-certified cardiologist with more than 30 years of experience in preventive cardiology, on staff at Lenox Hill Hospital and a Clinical Assistant Professor of Medicine at NYU Langone. Radwaner has said the goal is identifying hidden cardiovascular risk before symptoms appear, rather than waiting for chest pain to prompt evaluation. Advanced imaging does not replace ongoing primary care, and decisions about additional testing should involve a qualified healthcare provider.
