Virtual Contrast Supervision For Level 2 Diagnostic Tests: Guide Released

Contrast supervision is necessary to ensure patient safety during medical imaging procedures that involve the administration of contrast agents. Traditional protocols require a healthcare professional to be physically present in the imaging center to monitor the patient for adverse reactions to the contrast agent, but the Centers for Medicare & Medicaid Services (CMS) altered the definition of “direct supervision” to allow for supervision through real-time audio and visual interactive telecommunications. While the amended definition was intended for use during the pandemic, medical experts have found that the rule change benefited both patients and medical facilities alike by improving patient access and reducing operational costs, respectively.

According to ContrastConnect’s guide, traditional on-site contrast supervision has created persistent operational and financial pressures for imaging facilities nationwide. Unreliable on-site coverage and shortened operating hours stemming from limited personnel availability lead to patient care delays and lower revenue, while managing independent contractors creates additional HR work through onboarding, payroll, and compliance documentation. Furthermore, traditional on-site supervision is associated with higher staffing costs, with salaries, overtime pay, and administrative overhead often straining budgets without delivering proportional value.

Under the virtual model, imaging centers are less likely to encounter issues with scheduling conflicts and limited radiologist availability, allowing them to extend operating hours into evenings, weekends, and holidays to manage clinical backlogs. Facilities can also eliminate expenses associated with on-site personnel, lowering operational costs. Moreover, regulatory compliance and patient safety remain paramount under the virtual model as well, as real-time communication technology will allow experienced radiologists (MDs and DOs) to guide on-site staff in administering treatments in the event of adverse reactions.
